- Maintenance of Matriculation Form - for graduate students that need to "stop out" for general reasons, typically for one or two semesters. Students completing a thesis or dissertation should also complete this form if they need an extra semester to complete the written portion of their paper.
- Leave of Absence Request - for more serious cirsumstances (e.g. medical issues) that may require a leave from classes for an extended period of time.
